The appearance of the under-eye area is influenced by several anatomical features, including the position of the eye, underlying bone structure, soft tissue and the depth of the natural groove beneath the lower eyelid. A visible hollow does not automatically mean that tear trough filler will be appropriate.

For some patients, a defined tear trough creates a distinct depression and shadow between the lower eyelid and cheek. For others, the appearance may be predominantly related to naturally deep-set eyes or wider facial anatomy that should not necessarily be treated with filler.

At Derma Plus Medical Birmingham, we assess the area clinically before recommending treatment. This allows us to determine whether there is a suitable tear trough depression that could benefit from carefully placed dermal filler.

The aim of tear trough filler is to improve hollowing rather than eliminate every feature that contributes to a tired-looking eye area. Where a pronounced depression is creating shadow beneath the eyes, restoring volume can make the transition appear smoother and less sunken.

However, tired-looking eyes can also be influenced by pigmentation, skin quality, puffiness, vascular visibility and other anatomical characteristics that dermal filler does not directly treat.

Your consultation at our Birmingham clinic allows our medical clinicians to identify what is contributing to your concern and explain the degree of improvement that could reasonably be expected from tear trough filler.

The skin and soft tissues beneath the eyes are thin, meaning excessive filler can be more noticeable here than in areas with thicker tissue. Adding unnecessary volume may result in fullness or puffiness rather than creating the smoother contour the treatment is intended to achieve.

For this reason, tear trough filler at Derma Plus Medical Birmingham is performed conservatively. We use a maximum of 1ml during an appointment and may use less depending on the degree of hollowing and individual anatomy.

Where additional correction could be beneficial, reassessing the area after the initial filler has settled can be preferable to placing too much product during a single treatment.

It is common for facial anatomy to differ slightly between the two sides, and tear trough hollowing can therefore be more noticeable beneath one eye than the other.

Treatment does not necessarily require exactly the same amount of filler on each side. Instead, our medical clinicians assess the individual contours beneath each eye and determine where product should be placed to achieve an appropriate overall balance.

Some natural asymmetry will always remain, and complete symmetry cannot be guaranteed. This is discussed as part of your tear trough filler consultation in Birmingham so that expectations can be considered before treatment.

Before treatment, the under-eye area is assessed and the proposed approach is discussed with you. Medical-grade numbing cream containing lidocaine can be used to minimise discomfort, and the dermal filler itself also contains lidocaine.

Low-density filler is then carefully administered using a micro-cannula, with the amount determined by your anatomy and the degree of correction required. We do not use more than 1ml during an individual appointment.

After treatment, you may experience temporary swelling, tenderness or bruising around the under-eye area. Your clinician will provide appropriate aftercare guidance and explain what to expect while the filler settles.
